One warning, two meanings, is no warning at all

The airliner that flew on after a warning horn was heard correctly but understood wrongly, because the same sound meant two different things depending on when it was heard.

What happened

On 14 August 2005, Helios Airways Flight 522, a Boeing 737-31S registered 5B-DBY, departed Larnaca, Cyprus, on a scheduled service to Prague via Athens, carrying 121 people: 115 passengers and 6 crew. The previous evening, a ground engineer had carried out a pressurisation leak check and set the cabin pressurisation mode selector to manual. The selector was not returned to automatic afterward.

The incoming flight crew did not identify the incorrect setting. It went unnoticed through the preflight procedure, the after-start check, and the after-takeoff check: three separate opportunities, none of which caught it.

As the aircraft climbed through approximately 12,000 feet without automatic pressurisation, the cabin altitude warning horn sounded. On the 737 at that time, this horn was physically identical to the takeoff configuration warning used on the ground, a warning that can only be genuine while the aircraft is still on the runway. There was no separate indication to tell the crew which condition the horn was reporting. The captain radioed the airline's operations centre and reported a takeoff configuration warning, treating the sound as a ground-based nuisance rather than a cabin pressurisation emergency already under way.

The aircraft continued to climb. As cabin altitude rose, passenger oxygen masks deployed automatically, and the flight crew were progressively incapacitated by hypoxia. The aircraft reached cruise altitude and continued toward Athens on autopilot, entering a holding pattern near the airport with no response from the flight deck. Hellenic Air Force fighters intercepted the aircraft and observed the first officer slumped at the controls and oxygen masks hanging loose in the cabin. A member of the cabin crew, who held a commercial pilot licence, entered the flight deck shortly before the end of the flight, but the aircraft ran out of fuel before it could be brought under control. Both engines flamed out and the aircraft crashed into hills near Grammatiko, Greece, north of Athens. All 121 people on board were killed.

The investigation's direct causal chain was plain: the pressurisation selector was left in the wrong position, the crew did not identify the true nature of the warnings that followed, and hypoxia removed their ability to correct it before fuel ran out. The part of that chain aviation acted on hardest was the middle one: a single warning horn was carrying two unrelated meanings, sorted only by which phase of flight the crew assumed they were in. Under the load of a busy climb, the crew reached for the meaning they knew from years of hearing that horn on the ground, not the one that was actually true in the air. The investigation found no fault in their hearing, only in what the aircraft gave them to work with. The FAA required Boeing 737s to be retrofitted with a distinct visual warning for cabin altitude, so the two conditions could no longer be confused for each other.

On your site

A siren that means shift change in one building and evacuate in another. A light that means topped up on one piece of plant and fault on another. A radio call that means running late from one crew and something is wrong from another. Every one of these relies on the listener correctly guessing the context before they act on the signal, and a tired or distracted crew will default to the meaning they know best, not the one that happens to apply right now. A warning is only a warning if it means the same thing every time it is heard, regardless of who hears it or what they expect to hear.
